What should I bring with me when I visit the clinic?

Valid OHIP or other provincial coverage card
Another piece of government issued photo ID
All of your current medications in their bottles
The name and location of your family physician or nurse practitioner
The name and location of your preferred pharmacy
Debit or credit card to pay fees for services not covered by OHIP or other provinces

Yes we offer infant, child and adult immunizations and injections.

Do you have family physicians who are accepting patients

We provide most services that family physicians do. We do not sign up patients to a family practice. Our services are available to all.

Do you provide insured services to patients from other province?

Yes with a valid health card from another province. Quebec health cards are no longer accepted in Ontario but services provided for a fee for residents of Quebec.

Can you complete forms or sick notes?

Is your clinic wheelchair accessible?

Yes we have a wheelchair accessible entrance, washroom and registration kiosk.

How will I receive my test and imaging results?

Results will be communicated through a secure patient portal.
